In your example, in a 50/50, that combo would be x, x, then the last hit being either a low hit or a high. It's just like any other combo, except the setup can end two different ways.
To get more technical, you can have a 33/33/33 where you have three different ways to end a combo. I'm sure that there can be 25/25/25/25's and beyond, but I've never seen one (Ivan Ooze maybe, he's already broken as fuck)

It depends on the combo string or special move they do. You would have to look at frame data to figure out if it is punishable or not. By the way if you want to reply to someone and make it so they get a notification simply hit reply on their comment so they see you replied.
