Well, we know at the start of the story (in the scenes with Johnny and Sonya) he's fine, and at least up until he dropped off Takeda he was also fine. But since it seems like Variations are all story linked, something definitely happened between then and whenever he comes back into the story, so who knows.

My guess / hope is that its actually one of the spirits in his sword trying to take control of him.
